I understood the world was a painting when Lune gives her first dialogue and I'd pieced together that the fundamental elements of their universe are Lumina and Chroma and no one thinks it's weird to live in a city that looks and behaves like a surreal painting. pRenoir and pAlicia EXPLICITLY laying this out was a pretty good indicator, too.

The entire point of the story is that it's a tragedy against being seduced by the illusions of the world. Grief is the most powerful of those illusions. You can disagree, but you're disagreeing with Buddha and Jesus. Grief is "Significant" in the sense that you have decided to value it. Try analyzing why you do that and you'll start to understand that grief is a shortcoming that compromises you no differently from lust or greed.

"Dreams" in Bloodborne are separate dimensions that exist either parallel to, or overlapping with "our" reality. We can access these other dimensions in various ways, including sleep.
