I mean, people pretend like it's hard, but it really isn't, they just intentionally ignore the obvious: make the sex the scare. Like vampires back in their first novelization, the sex stuff and the violent murder bits were inextricably linked. It's not hard to pull off, though maybe that would change if you literally wanted a true porn game to also be scary. If you just want sexy and scary to work together though, then it's as simple as unifying them. Feeling powerless, weak, and on the verge of death/complete ruination is more integral to a 'scary' experience than anything else. It's not really hard to tie that in to sex, whether it's supposed to be in the context of supernatural monsters (succubi, vampires, etc), the fear of responsibility/growing up, or whatever else you can think of.
