They moved back the start date in EU3 and it was a fucking disaster. Every single game quickly became a fantasy scenario because the real historical players are too small to do anything (other than Castille, which usually goes on a conquest spree in Turkey)

Why are some countries like England or Lithuania called kingdoms and duchies whereas for others like Poland or Austria it just says their name without the title?

It is complicated. Some of the "independent" states of the era were actually in de facto permanent military alliances with stronger powers through treaties and marriage ties anyways until they were eventually absorbed.
