Depends on the denomination. IIRC catholics and ortodox have interpretations of hell not as an place but as a spiritual state. Protestants and especially evangelist really believe that hell is an actual literal place with actual demons and fire.

It's been a long time since I read the bible but I'm fairly sure it's designed by god, but maintained and lorded over by satan in the same way Earth is maintained and lorded over by Man; god has full control but we have free will and the ability to act out our own plans. Satan, who hates humanity because he blames them for being cast out of heaven (it was actually his own fault, he's just incapable of repentance which is why he's in hell still). In this way, hell is punishment even for satan, because he is trapped in a realm designed for humans and is beholden to his duties which involve enacting god's plan for humans.

Although you're encouraged to join the Church and to spend time with other christians, it's not mandatory.
All you need to enter Heaven is to put your faith in Jesus, to repent your sins and let him try to improve you.
