The group sex mechanics seem like they need some work as well, but that looks like something which would involve a massive overall. It works fine enough as it is.

I think a good way to do this is that once a play reaches a certain level of income or is regularly beating up Bailey's goons, Bailey offers the opportunity to work in a racket for her criminal organization in place of being a rentoid. If you were working as a prostitute, now you can use those skills to work as a pimp and pimp out the other orphans. If you were a thief, now you can run a pickpocket ring or a heist. The character will still very much be more of a head bitch than an actual boss, your underlings wouldn't respect you, they respect Bailey and Bailey says to listen to you. So you still have to keep Bailey happy or at the very least keep your underlings sexually relieved, it should maintain the submissive dynamic. Also you get to add management mechanics. Adding onto that, I think there should be more dominant content. I don't even do dom content in these kinds of games, but submissive has a sort of semantic relation with dominant. You need the latter to be present to accentuate the former more clearly. The game shouldn't focus on it, but the player should consistently come across dom options. If you're playing the game blind and just wandering around, it's pretty easy to end up in a situation where you don't have the required skills to get out but don't have an option to quit. So you just get stuck on the farm, or someplace else, clicking on the same options over and over while slowly building up a skill until you can eventually succeed in an escape. I get the whole bad-situation thing, but when you're literally spending hours just to grind up a horsesucking skill to eventually get the horses to like you, it gets really really boring. You're inclined to just reset to an earlier save or just not go exploring.

This anon is correct. The game has softlock states, where you can realize that its technically an expected ending for the player based on the events that happen, but then you can grind your way back into the sandbox and and continue like nothing happened.
If the game is supposed to be a spiral there could be a give in option that puts you in a new game state, maybe keeping some data about the "story", but resetting/replacing the player character.
Or the game could lean further in the direction its heading with allowing one of the relationships to permanently bail you out of the ophanage extortion trap. Currently the relationships have no conclusion, you can just run out of non-repeatable content and tgen continue on with the sandbox while ignoring things.
