None of this is new tho, I realized that in Dragon Age Origin.
There is no achievement in the game for completing the game in the highest difficulty, which I found odd because there is other "difficulty based" achievement such as not being KOed for x amount of time during the playthrough.
When I looked into it I found devs saying that they didn't want to make the players feel like they need to finish the game in that difficulty.
I'm sure that this shit started before that, I just realized it at that moment.

there is no more "intended experience", they just threw in a bunch of "modes" that just increase numbers with no other change and gave you a trillion sliders so you have to adjust the difficulty yourself because they spent no actual time doing so.
