E33 isn't a dream world any more than The Elder Scrolls series is. The world is self-perpetuating and the characters have all the qualities of real people. If the family fucked off and left the canvas alone, it would continue to exist and change.
In cliche IT WAS ALL A DREAM stories, nothing that happened was real and it doesn't matter once the dream wakes up.
This world is real to the people inside of it, and it doesn't require a painter anymore to exist (other than, I suppose, the animating force of Verso's soul splinter).
If God came down to Earth tomorrow and announced that he was real, he made the world, and he could end it whenever he wants, that would not make our reality a "dream". It would just mean that we have a creator.
The entire moral conundrum of the ending is predicated on this. It isn't just about a choice between, "give in to grief and live in a fantasy" vs "get over it".
Verso's ending isn't just coming to terms with grief and moving on, it also means destroying an entire world of thinking, feeling people.
It's pretty fucked.

However, I didn't like the twist, I felt like it was a bit of a cop out, when other games have done this twist I also felt like it was a bit of a cop out, and then after it did a twist that I did not like, it raised a bunch of questions that leaves the player to speculate, and I'm fine with the speculations I've made (except for a couple things which seem like oversights or DLC bait, but they said no planned DLC, so...), but the entire thing has been soured because I felt like the lead up to Act III was a bait and switch, and not in a positive way. It was "oh... damn I thought it was gonna be better than that"
