You know this bit at the start of the game? It's Dess. She's literally the first character we're shown in the game and we don't realise it because she's shown completely covered in darkness. But we are able to see her red nose. Because it glows just like in the rudolf christmas song.
Go ahead and look up Noelle's sprite. Her nose is red (as shown in her overworld sprite) and shaped like a downwards pointing triangle. It's not a stretch that her sister's nose could be like that but with two slight bumps at the top to make it heart shaped.
The slight hovering you see? That's actually Dess breathing. She's doing the thing sprites often did in undertale battle screens where the head and body would slowly bob up and down to depict breathing. But because all we can see of her is her glowing nose it looks like it's hovering in the air.

Ive been saying the same thing for a while, the ANGEL's HEAVEN is what happens when Light overtakes Shadow. The ROARING is what happens when Shadow overtakes light. They are very explicit about this. We also have seen the ANGEL itself and it shares traits with the Titans namely the fact that it looks like a save point. Also it looks like the ANGEL's HEAVEN Light is raining from above in the legend while the fountains ascend from below. There might be someone in the LIGHT world doing the opposite of the Knight.

And Dess's addition may have been as late as 2017 if the changes to the Alarm Clock dialogues are anything to go by.

A really big thing is that the whole FIGHT/SPARE dichotomy being carried over from Undertale is something he did just because he knew people would expect it coming from Undertale, especially after he decided to amalgamate the game with Undertale.
That dichotomy represented by Susie and Ralsei is the main thematic element of Chapter 1, so that was a pretty massive change. Beyond just Chapter 1 this probably also motivated Susie becoming a "mean girl" (Toby describes her original character as being inspired by Maya Fey from Ace Attorney) and Susie being a bully is the foundation of her relationship with Kris and Noelle, so this was a major part of the game actually taking shape post-Undertale.
It's hard to say if the Weird Route was also already being planned at this point (if you're thinking about fight/spare you would naturally consider another take on Genocide) or if that didn't start until more toward Chapter 2's development, when the bigger picture was more clear (and the idea of having major story divergence became more feasible with an actual development team, after Chapter 1 was so difficult).

Based on the concept art that Toby shared it doesn't seem like the Queen of Spades that Kanotynes designed was really the starting point for Queen. Kanotynes's design was basically just Lancer again, or a halfway between him and King.
It seemed like Toby was prepared to drop the association between Queen and King/Lancer entirely, the snake/Medusa theme was explored pretty thoroughly to the point of giving her a green/purple color scheme. It looks to me like something that they worked their way back to, rather than it being the plan from the start. But this is all just speculation.

90% likely he had the destination in mind, a few main characters, and the general outline of the plot. No major specific Dark Worlds, bosses, or whatnot. Probably most of the town was devoid of actual characters, which is why he pulled then from Undertale.
Anyone who's ever made anything knows that plans do change over time. He's probably got an idea of the ending still, but he might be changing motivations of villains, exact routes, important charactersetc. Wouldn't be surprised if he started with only Kris, Susie, Noelle, and Ralsei as party members, but he got the idea to have more. Heck, the way we were talking about Snowgrave might've inspired routes for other characters, instead of just keeping it at one.
This will be easy to confirm with the next schizoboss, but I think the shadow crystals each have a connection to a UT boss character that uses a colored soul. Seam/Jevils was stolen, probably from Asgore or asriel in one way or another, or maybe Asriel played with the toy when he was a school kid. Since it seems to be working backwards with Mettaton, this would probably put Green soul next which fits with requiring the Mantle (since it works with shield mode and the Franklin badge inspired deflection of electric/holy damage). There could be an additional pattern with the boss also having the title of "the Undying" like spamton NEO
