Back in this era gaming and anime had a large crossover. Even in the original MGS era it was sort of the "weird cousin" of the American wargames. Sure, you'd get some people who played it but it wasn't a major breakthrough hit to people who otherwise never played vidya or only shooters and sports etc. Fushigi Yugi itself was not so much the issue, but rather that there were plenty of "pretty men" in anime who were none the less understandable as "men" and in many cases were directly shown to be both interested in women and highly desired by them, so it wasn't like these were exclusively "joke" characters or those specific yaoi tropes or whatever even to more casual anime / gaming audiences. I'll grant my experience may have been earlier and I had access to fansubs back when they were giant VHS yellow block lettering, but its not like the concept itself was totally alien to anyone who was even somewhat familiar with Japanese media.

I don't think he really was, as other big titles (including FF7 and FF8) had pretty characters. As far as Raiden being masculine, he's well within what was a common Japanese trope for manliness - the "long haired handsome swordsman" ; Raiden even gets the fucking HE blade late in MGS2 to kind of drive the point - wasn't outside of familiarity to many game players and anime watchers of the time. As far as the president cupping his junk, that was a joke no different than Otacon or Johnny Sasaski's bathroom humor, but also all the stuff involving Olga.
