I have anecdotal stories from a friend that worked at Bethesda that Todd would just stop by and shoot the shit for several minutes every so often. My friend thought it was nice and friendly but he's also a workaholic so would get anxious about having to talk to the Todd instead of working.
I'd definitely agree that Todd seems like a good dude who's just decided to play things too safely after seeing what Morrowind did to his coworkers. They were in do or die mode and had to crunch like crazy for it and it nearly killed them (as a company and as individuals. I think Ken Rolston or Goodall or one of them put on like 60 pounds during it). I don't blame him for playing it safe, especially since he seems to want to take good care of his employees, but Bethesda plays it too safe with their own games. there are a lot of mountains, the impression you get is that all the mountains you see can be walked/climbed on. That everywhere you see is a place you can go, but really its just the one mountain he was talking about and most of the rest are effectively just artwork.
Technically its not a lie, but its misleading on purpose which is dishonest. The marketing for the game used A LOT of innuendo to make you think you were getting a larger and deeper game than you got. They also showed off the golden claw quest before release and showed how you used to item menu to look at items to figure out puzzles in the game world. It made you think that there was going to be some secrets in the world with some interesting puzzles involving item inspection. In reality there was just the claw puzzles that used that feature and the game explicitly tells you to look at it, in case you missed their advertisement.
skyrim was at least a little better than oblivion because you could find unique spell books out in the world, and if you werent a caster there were dragon shouts too, so the explorations was a step above oblivions where you did gates for sigil stones and ran the same marauder dungeon with a good playout over and over instead of looking for new dungeons because you knew there was nothing unique in different dungeons if a quest isnt pointing you to go there.

Most people agree that being able to ignore combat in a DOOM game would be fucking cancer. If you make it so a player can ignore any given mechanic in the game then they're disconnected and isolated. They don't interact. There's no connection and the game feels disjointed. Which is a huge problem in Starfield. It's a space game where space combat is optional and you can't even fly your ship.
But, yes, one of their biggest issues is that they think brevity is the soul of wit for game design in a genre focused on having a ton to do.

It ruined some of the best people Todd had working under him and they left the company right after.

It is what led to a slow and gradual decline in the pre-existing mechanics of the later games moving forward, and a lack of proper development of the engine to support the vision Bethesda marketed itself on with its big open worlds.
They would advance the graphics, throw in some new ideas like basebuilding and the like, but would get simpler with everything else that was already around.
The engine would be updated for more memory but the optimization was always so bad they never moved beyond the problem of Vivec being too big to render properly in Morrowind.
They only approached that problem properly ONCE across their games, and it was with the capital wasteland in fallout 3, which ultimately was the same work around used for Morrowind, but executed in a better fashion.
Skyrim litterally runs on the same engine as civilization IV.

The difference is that they made DC a bit more interesting than the city of Vivec was.
Its not a big improvement, but a slight improvement is better than NOT improving.

DC doesn't really function as a city though.
That is only because they made it as a wasteland by choice.
The core issue preventing them from making larger cities is that they cannot load a large city in one go, with all of the NPCs and models.
DC is so chopped up that it is EASILY capable of supporting that, but its meant to be largely a free-for-all zone, so naturally they were not going to populate it like a true city.
Instead it contains a couple mini-cities with the ghouls, brotherhood outcasts, talon company, and rivet city.
They used the newfound freedom to make a labyrinth out of things to encourage exploration.

I'm not sure how much they solved that issue with the engine considering the strip in new vegas also ended up having to be split into two parts.
